| Assessment | ASSESSMENT_NAME_CODE | Name of the assessment that is assigned to the screening home page configuration. | No |
| Start Date | CURAM_DATE | First date of the period during which the assessment is assigned to the screening home page configuration. | No |
| End Date | CURAM_DATE | Last date of the period during which the assessment is assigned to the screening home page configuration. | No |
| Status | RECORD_STATUS | The status is "active", unless the record has been deleted, in which case, the status is "canceled". | No |
