Description
This page allows you to create a new enrollment details record. For the CHIP program, the Department accepts applications for enrollment at times when sufficient funding is available to justify enrolling more individuals. The Department limits the number it enrolls according to the funds available for the program by having a specific enrollment period. Applications are only accepted during this time unless the child meets an exception. These 'open enrollment' periods vary between states.
