Fee For Service Type	FEE_FOR_SERVICE_TYPE	This field displays the type of fee for service being provided, e.g. Dental Service, Medical Service.	No
State	STATE_CODES_CODE	This field displays the state in which the fee for service option is provided.	No
County	ISP_COUNTY_CODE	This field displays the county in which the fee for service option is provided.	No
Start Date	CURAM_DATE	This field displays date from which the fee for service option is provided by the county.	No
End Date	CURAM_DATE	This field displays date on which the county ceases to provide a fee for service option.	No
Status	RECORD_STATUS_CODE	This field displays the current status of the fee for service, e.g. Active.	No
